The aim of the present investigation was to describe and to classify significant ethical problems encountered by the members of the staff during the daily clinical work at a hospital medical department. A set of definitions was prepared for the purpose, including the definition of a 'significant ethical problem'. During a three month period 426 inpatients and 173 outpatients were admitted. Significant ethical problems were encountered during the management of 106 in-patients (25 per cent) and 9 out-patients (5 per cent). (...) No significant difference was found between the frequency of ethical problems in female and male patients, but a positive correlation was noted between the number of problems and the patients' age. The problem types were classified according to a problem list. The results of this investigation suggest that greater attention must be paid to discussions about ethical problems among doctors and other categories of health personnel and that, among others, medical students ought to be taught the analysis of ethical problems. Nursing policy and healthcare reform are focusing on two, interconnected areas: person‐centred care and fundamental care. Each initiative emphasises a positive nurse–patient relationship. For these initiatives to work, nurses require guidance for how they can best develop and maintain relationships with their patients in practice. Although empirical evidence on the nurse–patient relationship is increasing, findings derived from this research are not readily or easily transferable to the complexities and diversities of nursing practice. This study describes a novel methodological approach, called (...) holistic interpretive synthesis (HIS), for interpreting empirical research findings to create practice‐relevant recommendations for nurses. Using HIS, umbrella review findings on the nurse–patient relationship are interpreted through the lens of the Fundamentals of Care Framework. The recommendations for the nurse–patient relationship created through this approach can be used by nurses to establish, maintain and evaluate therapeutic relationships with patients to deliver person‐centred fundamental care. Future research should evaluate the validity and impact of these recommendations and test the feasibility of using HIS for other areas of nursing practice and further refine the approach. (shrink)

This is not news. What is news and what is becoming more apparent to those who examine the issue is that the alternatives Strauss presents in the above quotation are not mutually exclusive. Indeed, as I and my colleague, Douglas J. Den Uyl, have recently observed: Making the individual the moral center of the universe does not require that one accept nominalism, mechanism, or hedonism, nor does accepting essentialism, teleology, and eudaimonism... require rejecting individualism. It is possible for the fulfillment of the individual to be interpreted in terms of the requirements for human wellbeing. There can be a view of the ego or self that is neither otherwordly nor Hobbesian, but Aristotelian. Further, the achievement of man's natural end need not be interpreted along Platonic lines. There is no such thing as the flourishing of "man." There is only the flourishing of individual men. The human good neither exists apart from the chokes and actions of individual human beings nor independent of the particular " mix " of goods that the individual human being must determine as appropriate for his circumstances. *This essay is based on a paper presented at a meeting of the American Association for the Philosophic Study of Society, American Philosophical Association (Eastern Division) convention, December 29, 1991 in New York City. RASMUSSEN Strauss's dichotomy betrays a disturbing tendency, often found among proponents of natural right and natural law, to reify the concept " natural end " and make it some good that competes with the good of individual human beings.1 David L. Norton in his recent work, Democracy and Moral Development, has also noted the nonexclusivity of Strauss's alternatives. Conditioned as we are by modernity's conception of the individual, the question of replacing avarice as the thematic motivation in lives of persons is likely to leave us at a loss. What else could possibly serve? The purpose of this book is to propose the eudaimonistic answer, and that answer is love in the meaning of Eros. Thus understood, love is not exclusively or primarily interpersonal; it is first of all the right relationship of each person with himself or herself. The self to which love is in the first instance directed is the ideal self that is aspired to and by which random change is transformed into the directed development we term growth. When the ideal of the individual is rightly chosen, it realizes objective values that subsisted within the individual as innate potentialities, thereby achieving in the individual the self-identity that is termed "integrity" and that constitutes the foundation of other virtues.2 Norton desires to reconceptualize the individualism historically associated with classical liberalism so as both to retain the gains of classical liberalism and to overcome its moral minimalism. According to Norton, the crucial problem with the conception of individuality historically associated with classical liberalism is that it is "non-developmental," or rather, there is only development within a single stage, namely, for self-preservation. There is nothing more. There is no standard of self-perfection by which to distinguish satisfaction of desire from satisfaction of right desire -no way to distinguish Socrates's satisfaction from that of a fool's. According to a recent challenge to Kratzer's canonical contextualist semantics for deontic modal expressions, no contextualist view can make sense of cases in which such a modal must be information-sensitive in some way. Here I show how Kratzer's semantics is compatible with readings of the targeted sentences that fit with the data. I then outline a general account of how contexts select parameter values for modal expressions and show, in terms of that account, how the needed, contextualist-friendly readings might plausibly (...) get selected in the challenge cases. In the second part some comments are presented on the meaning of the developments and the possible causes. Current orthodoxy in research ethics assumes that subjects of clinical trials reserve rights to withdraw at any time and without giving any reason. This view sees the right to withdraw as a simple extension of the right to refuse to participate all together. In this paper, however, I suggest that subjects should assume some responsibilities for the internal validity of the trial at consent and that these responsibilities should be captured by contract. This would allow the researcher to impose a (...) penalty on the subject if he were to withdraw without good reason and on a whim. This proposal still leaves open the possibility of withdrawing without penalty when it is in the subject's best interests to do so. Giving researchers recourse to legal remedy may now be necessary to protect the science, as existing methods used to increase retention are inadequate for one reason or another. (shrink)

As the usual regulatory framework did not fit well during the last Ebola outbreak, innovative thinking still needed. In the absence of an outbreak, randomised controlled trials of clinical efficacy in humans cannot be done, while during an outbreak such trials will continue to face significant practical, philosophical, and ethical challenges. This article argues that researchers should also test the safety and effectiveness of novel vaccines in wild apes by employing a pluralistic approach to evidence. There are three reasons to (...) test vaccines in wild populations of apes: i) protect apes; ii) reduce Ebola transmission from wild animals to humans; and iii) accelerate vaccine development and licensing for humans. Data obtained from studies of vaccines among wild apes and chimpanzees may even be considered sufficient for licensing new vaccines for humans. This strategy will serve to benefit both wild apes and humans. (shrink)

The principle of the child's right to an open future was first proposed by the legal philosopher Joel Feinberg and developed further by bioethicist Dena Davis. The principle holds that children possess a unique class of rights called rights in trust—rights that they cannot yet exercise, but which they will be able to exercise when they reach maturity. Parents should not, therefore, take actions that permanently foreclose on or pre-empt the future options of their children, but leave them the greatest (...) possible scope for exercising personal life choices in adulthood. Davis particularly applies the principle to genetic counselling, arguing that parents should not take deliberate steps to create physically abnormal children, and to religion, arguing that while parents are entitled to bring their children up in accordance with their own values, they are not entitled to inflict physical or mental harm, neither by omission nor commission. In this paper, I aim to elucidate the open future principle, and consider whether it is applicable to non-therapeutic circumcision of boys, whether performed for cultural/religious or for prophylactic/health reasons. I argue that the principle is highly applicable to non-therapeutic circumcision, and conclude that non-therapeutic circumcision would be a violation of the child's right to an open future, and thus objectionable from both an ethical and a human rights perspective. The authors argue that while meaning holism makes massive error possible, it does not, as Donald Davidson fears, threaten interpretability. Thus they hold, in opposition to Davidson, that meaning holism need not be constrained by an account of meaning according to which in the methodologically most basic cases the content of a belief is given by the cause of that belief. What ensures interpretability, they maintain, is not that speakers' beliefs are in the main true, but rather that beliefs have (...) the contents they do because of events others can in principle identify and describe. Cells are the fundamental building blocks of organisms and their organization holds the key to our understanding of the processes that control Development and Physiology as well as the mechanisms that underlie disease. Traditional methods of analysis of subcellular structure have relied on the purification of organelles and the painstaking biochemical description of their components. The arrival of high‐throughput genomic and, more significantly, proteomic technologies has opened hereto unforeseen possibilities for this task. Recently two reports(1,2) show how much can be (...) gleaned from the combination of analytical centrifugation, mass spectrometry and advanced statistical techniques focused on a high‐throughput analysis of the content and organization of plant and animal cells. The results reveal intriguing possibilities for the future and the possibility of mapping much of the known proteome onto our current map of the cell.
